S28 ENDOSCOPIC ULTRASOUND / VOLUME 6 / SUPPLEMENT 1 / AUGUST 2017 Mitsuyoshi Honjo, Mitsuru Fujita, Kenjiro Yamamoto, Yukitoshi Matsunami, Yasutsugu Asai, Takao Itoi Tokyo Medical University, Tokyo, Japan Background and Objectives: Endoscopic ultrasoundguided transluminal drainage (EUS-TD) and sequential direct endoscopic necrosectomy (DEN) for pancreatic fluid collections (PFCs) have been reported as a useful treatment. Recently, a new dedicated biflanged metal stent (BFMS) matched to the PFC condition has been developed. Herein, we show the prospectively evaluated clinical outcome of this new BFMS for the treatment of PFCs. Methods: EUS-TD using new stent was performed in 17 patients for PFCs (4 pancreatic pseudocysts; 13 walled-off necrosis). When clinical resolution could not be achieved within a few days after EUS-TD, DEN was performed the following day. Results: New stent was deployed successfully with a median procedure time of 16 min and with no procedurerelated adverse events in all the patients. DEN (n = 5) and/or additional drainage procedure (n = 6) through the stent were achieved in all the patients in whom they were attempted (8/8). Stent dislocation during DEN was not observed in any patients, but in one patient, spontaneous stent migration was observed. Two WON patients died from pseudoaneurysm rupture occurring between the endoscopic necrosectomy sessions and from multiple organ failure although hemostasis was achieved by coil embolization. The PFCs in the other 15 patients completely resolved. Conclusions: The new BFMS is technically feasible and safe for the treatment of PFCs. DOI: 10.4103/2303-9027.212310

Endoscopic ultrasound-guided pancreatic duct drainage: technical approaches to a challenging procedure.
Endoscopic ultrasound-guided pancreatic duct drainage (EUS-PDD) is an effective treatment modality for pancreatic pathologies, including chronic pancreatitis, major or minor papilla inaccessible via endoscopic retrograde cholangiopancreatography, and postsurgical pancreaticoenterostomy stricture [1,2]. Technically, however, EUS-PDD can be very difficult and is potentially associated with signif...
متن کاملEndoscopic therapy in chronic pancreatitis: current perspectives
Endoscopic therapy in chronic pancreatitis (CP) aims to provide pain relief and to treat local complications, by using the decompression of the pancreatic duct and the drainage of pseudocysts and biliary strictures, respectively. This is the reason for using it as first-line therapy for painful uncomplicated CP. The clinical response has to be evaluated at 6-8 weeks, when surgery may be chosen....

Endoscopic ultrasound (EUS) was developed as a diagnostic modality but rapidly gained a role for a variety of therapeutic applications. Endoscopic ultrasonography (EUS)‐guided cholangio pancreatic drainage has been reported as an alternative to surgery or percutaneous transhepatic cholangiography if endoscopic retrograde cholangiopancreatography (ERCP) is unsuccessful. Its concept includes EUS‐...

Endoscopic retrograde cholangiography (ERC) is the most common method employed for drainage of malignant biliary obstructions. When ERC fails, bypass, either percutaneously or surgically, is available; more recently, endoscopic ultrasound-guided biliary drainage (EUS-BD) has been described [1–3]. Most endoscopists perform EUS-BD using a rendezvous approach in which the bile duct is accessed usi...

Endoscopic ultrasound (EUS)-guided pancreatic duct access in patients with an inaccessible papilla or failed endoscopic retrograde cholangiopancreatography (ERCP) has been reported previously [1, 2]. This technique consists of the EUSguided rendezvous technique, and EUSguided pancreatic duct stenting via the stomach (pancreaticogastrostomy), or the duodenum (pancreaticoduodenostomy) [2]. Howeve...
